In the near future, Berlin's Neukölln district has collapsed due to a breakdown of the public health system. Dr. Ketel, an underground doctor without a license or practice, operates illegally, treating people in courtyards and streets wherever he is needed. He steals medicine from pharmacies to provide for his patients. Despite his efforts, he faces numerous challenges and defeats that drain his strength.
